Enrollment

Enrollment Procedure

Tours of the facilities are required prior to beginning the enrollment process.
— Upon completion of a tour, the parent/legal guardian may place their child on our waiting list.

(No fee for waiting list)

— Director will call or email when space becomes available within a program.

— A $75 registration fee is required to secure the child’s place.

— Parent/legal guardian and the director will coordinate how to collect the fee and distribute the enrollment paperwork.

Forms must be completed and submitted to the Program Director prior to the child’s first day of attendance.

— The information in these forms will be shared with other caregivers only as needed to meet the needs of the child.

Enrollment Documents

1. Child Health Assessment - signed by the child’s physician or certified registered nurse practitioner (CRNP)

a. These are updated according to the schedule below.

i. Every 6 months for an infant or young toddler (1 y/o).

ii. Every 12 months for an older toddler (2 y/o) and preschool child (3 & 4 y/o)

iii. School Age Children

b. Immunization Exemptions Form

i. If you are exempt from any or all immunizations, DHS requires a hand-written statement concerning your exemption.


2. Childcare Emergency Contact Information – signed by a parent or legal guardian for each child enrolled.

a. Updated every 6 months or whenever the information changes.


3. Childcare Agreement – completed by a parent or legal guardian.

a. Updated every 6 months or whenever the information changes.


4. Get to Know You Form – This provides the childcare staff with background information about your child to aid in their care.


5. Publicity Waiver – This will give parents the option to have their child’s picture included in any publicity pictures that may be taken.


6. Legal Documents – (if applicable) We require parents/guardians having children under joint custody in divorce/separation arrangements, to provide the director with a copy of any pertinent legal documents. This information must be received prior to your child’s first day of attendance.


7. Special Care Plan – (if applicable) When the parent or legal guardian informs the facility staff that a child has a disability, a special care plan will be completed by a parent or legal guardian and/or health care provider for that child.

a. A parent or legal guardian may be asked to authorize release of information from providers of special services to help the childcare provider coordinate the child’s care.

b. Bethany Childcare Programs will use IEP or IFSP written plans, and/or special needs assessments, with family permission, to inform individualized instruction.

c. Bethany Childcare Programs will offer meeting spaces for IEP or IFSP meetings.

i. The child’s teacher, parents, EI specialists, and administration are all to be present at the meeting.
